TO MAKE YOUR FIRST VISIT TO OUR OFFICE EASIER, PLEASE:

 

  1. If you would like, you may have a copy of your medical records forwarded to our office from your previous clinic or physician’s office. Usually the last 3 years of records should be sufficient.   Complete the release of medical information form and send it to your previous clinic or physician’s office.  If the records arrive at least two weeks prior to your first visit, the physician will usually be able to review the records before your visit.  If your records arrive only shortly before your visit, the doctor will review portions of it together with you at the visit and schedule you to return after she has fully reviewed your records.  If the records are not received prior to your visit, you will be scheduled to return after any needed records are reviewed.  Physician time to review an average amount of medical records is a courtesy and free of charge but if your medical records necessitate more than the average time to review, there will be a charge for the extra physician time to look over your medical records.   

 

  1. Print out and complete the registration forms and health questionnaire.   Please bring the completed forms to your appointment.

 

  1. Bring all prescription medications to your appointment in their original pharmacy bottles.

 

  1. Bring your insurance card(s) so that we can make a copy of it.  This information will be used for the billing form that we will give you to send to your insurance company.

 

  1. Carefully check your insurance policy regarding coverage and reimbursement rates.  Contact your insurance regarding coverage if needed.  If you have any questions regarding our office payment policy, please call us.  Payment is expected at the time of service.  We prefer checks but credit cards and cash are also accepted.

 

  1. Please arrive 15 minutes early for your first appointment so that we can complete the registration process.

 

  1. If you need to cancel or reschedule your new patient appointment, please give us 1 week notice so that we can accommodate other patients who are waiting.
