Preparing for an injection appointment:

If you or a member of your household have any symptoms below in the last 14 days, do NOT come to the office.  Call us to discuss first.

1.     Contact with or potential exposure to a COVID-19/corona virus patient:

 

2.     Fever/Chills         

 

3.     Sore throat/HA/runny nose/sneeze:    

 

4.     Body aches

 

5.     Rash including on toes

 

6.     Loss of smell or taste

 

7.      Cough

                   

8.      Shortness of breath

 

9.     Diarrhea              

 

10.  Nausea or vomiting

 

11. Abdominal pain

 

  

Day of visit:

 

Before you leave the house:

·         Use the bathroom before leaving the house to minimize contact with other patients in our office 

·         Bring your cell phone

·         Bring your face mask –wear your most protective mask.  An N95 without a valve if you have one.  Otherwise, a surgical ear loop mask.  If you only have a cloth mask, a KN95, or N95 with a valve, we will give you a surgical mask to put on top of your mask. 

·        Wear a tank top or loose short sleeve shirt that has sleeves that roll up easily

Upon arrival outside in front of office

·        Call us from your car so that we know you are here.  We are not having patients use our waiting room in order to avoid infecting others.

 

·        If we do not answer the phone, please ring the doorbell and wait outside. 

 

·        Injections will be administered outside our office.  We will meet you outside when we are ready.

 

During your appointment:

·         Please keep your facemask on. 

After your appointment

 

·        Payments will be processed using the credit card on file.

 

·        Any needed insurance forms will be mailed to you
